Plan availability, rate variables, and premium calculations are dictated solely by localized underwriting guidelines, geographic zip codes, and carrier network maps.

TCPA Contact Authorization: By selecting "Submit Health Intake" or providing your telephone number and email address on any custom intake module within lopezadvisory.agency, you certify that you are the primary subscriber of the provided account numbers. You extend explicit, prior express written consent authorizing a licensed independent broker representing Lopez Advisory to reach out to you directly using voice calls, text messaging (SMS), or secure electronic mail using the provided data coordinates.
