When using our digital intake tools or request forms, you certify that:

• All submitted name, age, zip code, contact details, and medical profile parameters are accurate, current, and complete.
• You are authorized to submit data for yourself or the designated dependent on the form.
• You will not submit false, duplicate, or automated bot inquiries.

3. Telecommunication & Consent Policy

By entering your contact number or submitting an online form on lopezadvisory.agency, you authorize Lopez Advisory to contact you directly via phone call, email, or SMS text message. 4. Regulatory Disclosures & Non-Affiliation

In accordance with state and federal regulations, you acknowledge that:

• Government Non-Affiliation: Lopez Advisory is a private agency and is not affiliated with the United States Government, the federal Medicare program, the Centers for Medicare & Medicaid Services (CMS), or Connect for Health Colorado.
• Broker Fee: We do not charge broker fees to the consumer. Our advisory operations are carrier-compensated.

5. Limitation of Liability

While we make every effort to display accurate parameters regarding policy categories and coverage maps, Lopez Advisory is not liable for errors in third-party carrier rates, network modifications, or policy claim rejections issued by the carrying insurer. The final policy contract governs all coverage benefits and parameters.

6. Changes to Terms

We reserve the right to modify these Terms of Service at any time. Your continued use of the website or our advisory services constitutes acceptance of the modified Terms.
